Go to FormTo support the health and safety of students in Bermuda with asthma, the Bermuda Department of Education, Department of Health, Bermuda Hospitals Board Asthma Education Centre, and Open Airways invite parents/guardians to complete the Bermuda Student Asthma Registry form. This initiative aims to ensure appropriate care for children (ages 4-18) managing asthma in schools. Please Note: A separate form is required for each child with asthma. If your child does not have asthma, please exit the form.
